Job summary This position rotates between the community therapy team (working with patients in their own homes) and the 'in-reach' rehabilitation beds team (supporting rehabilitation and discharge planning in x3 care homes). You will work alongside Physiotherapists' and Occupational therapists to support carrying out exercise programme's and functional activities to support patients with their rehabilitation. This post involves working with patients with a wide variety of pathologies and medical conditions, who have recently been discharged from hospital and require additional time to help improve their mobility and independence before returning home. Due to the domiciliary nature of this post a full UK driving license and access to a car is desirable. The post is subject to successful interview, references, DBS clearance and Occupational Health checks.
