Job Summary
The Driver and Support Co-ordinator will be responsible for driving the van to required locations, ensuring the van is clean and fully stocked to support the aims of the SWP ODN service across Devon, Cornwall & Somerset. The aim of the Community Health Van service is to target hepatitis C high prevalence areas of at-risk, hard-to-reach groups - people who are homeless, using drugs, and using alcohol (currently and previously) - and deliver services directly into their community. This will increase testing of undiagnosed people, especially if they are unaware of risk factors or deterred by stigma, and support engagement of people who have been diagnosed but not yet accessed care. The service will also offer a holistic approach to people accessing the van, providing routine health checks, support and advice on safer injecting practices along with referrals for help and support to drug and alcohol rehabilitation courses and treatment if required.
Main Duties of the Job
* Manage and coordinate, in partnership with the South West Peninsula Hepatitis C Operational Delivery Network, all aspects of driving, maintenance, and consumable stocking, cleaning, and removing waste from the van (including clinical and dirty waste).
* Coordinate logistics, paperwork, and stock for outreach sessions, ensuring that the publicity of events is timely and any other activities required to run a professional and bespoke service are in place.
* Work with the team to develop links with service providers (including homeless hostels, local drug and alcohol services/teams) to inform them of mobile clinic activities and encourage their participation.

Job Responsibilities
* Be responsible for driving, parking, maintenance, servicing of the community health van. Additionally, all operational matters concerning the van including security of van and contents and regular stock checking and cleaning.
* To co-ordinate and be responsible for the day-to-day operational management of the mobile outreach van.
* Undertake promotional activities to publicise the mobile outreach service to ensure the service is well known and well attended.
* To support the work of the Hepatitis C Trust and the SWP ODN by developing knowledge of hepatitis C, and act as a source of advice and support to people accessing the van.
* Oversee and implement administrative, recording, and monitoring systems for the community health van ensuring they are in line with the SWP ODN's internal policies and procedures.
* To take full responsibility for the community health van, ensuring that all documentation is up to date.
* Work in partnership with local hospital staff and volunteers to ensure the community health van is fully staffed and appropriate patient pathways are in place.
* Act as a professional lead member for the community health van and main point of contact with public and site staff at testing sites.
Person Specification
* UK Driving Licence - Category C1 (Essential)
* Demonstrable experience of working collaboratively with external organisations (Essential)
* Demonstrable experience of working in the health service (Desirable)
