Our public sector client based in South Tyneside requires a Hospital Discharge Coordinator/Administrator to join their busy Integrated Hospital Discharge Team as soon as possible The role is office based and will be a temporary position, running through until the end of March 2025 initially. The hours of work are usually Monday to Friday, with some flexibility on start and finish times. There may be occasions where some shift work is required over Bank Holidays and including weekends, which will be on a rota basis. The hourly rate for this role is £13.69 per hour. The duties of the role will include: - · Supporting operational social care teams with ensuring people return from hospital in a safe and timely manner · Responsible for the coordination and management of a daily position statement and circulate daily · Build relationships with all teams, partners and providers · Challenge decisions, delays and queries which impact the flow of hospital discharge · Ensure there is access to care and support if needed on the day of discharge and work with partners to reduce delays · Work in accordance with safeguarding policies · Oversight and management of daily tasks, phone calls and email enquiries received into the Hospital Discharge Team · Manage daily operational queries including those of a complex or potentially contentious nature, in a way that is customer focus · Prepare and present reports To be considered for this role you should have a strong background within office administration and have excellent IT skills. The successful candidate will have the ability to maintain resilience whilst working under pressure in a fast-paced service, ability to work autonomously as well as within a team, flexible to adapt to change as the service demands and open to engage in peer support and challenge.
