Job Title: Qualified Adults Social Worker – Hospital Discharge Team
Location : Surrey
Rate : Up to £33.50 per hour
An exciting job role has recently become available at Surrey County Council. They are looking for a new member for their Hospital Discharge Team.
What will your responsibilities be?
As an Adult Social Worker for the Hospital Discharge Team, your responsibilities will include conducting thorough assessments under the Care Act to determine patients’ needs and support requirements. You will coordinate services and resources to ensure smooth transitions for patients leaving the hospital. Additionally, you must demonstrate expertise in adult social care legislation and practices to provide effective support and advocacy for patients.

Qualifications and Experiences:
* Experience conducting thorough assessments under the Care Act to determine patients’ needs and support requirements
* Experience coordinating services and resources to ensure smooth transitions for patients leaving the hospital
* Able to demonstrate expertise in adult social care legislation and practices
* Social Work England registration
* Eligible to work in the UK
* Hold a full UK driving licence
* Post-qualified experience
* Degree level or equivalent in Social Work
