As a skilled Care Coordinator, you will manage a caseload of care packages and understand the varied nature and fast pace of the role, along with all elements of associated care delivery. You will apply your communication and relationship building skills to effectively manage rotas for your care teams, respond efficiently to any day-to-day changes, and act as the first point of contact for service users, their families, and other healthcare professionals. No matter which care setting you have obtained your Care Coordination experience in, you will be aware that no two days are the same, so you will be able to evidence your adaptability and aptitude to work well under pressure, using your initiative and thinking on your feet. This is an office-based role, Monday to Friday, 9am – 5pm but occasionally, travel across the Merseyside region will be required. Therefore, applicants must hold a Driving Licence and have access to a vehicle. You must also be committed to supporting the out of hours on-call service, which is covered on a 1 weeknight per week and 1 weekend in 4 rotational basis across the team. We're offering a competitive salary between £26,000 - £28,000 per annum, plus payment for on-call. There is also the opportunity to earn a bonus of up to £2,000pa.
