What you will do:
1. Provide specialist clinical pharmacy services targeted at optimising patient care in specific clinical and operational practice areas.
2. Support the medicines-related care of patients and advance the development of medicines management services.
3. Engage in training, education, and research, contributing to the overall enrichment of the Trust’s pharmacy services.
4. Actively participate as part of specific wards, units, and clinical teams, ensuring the safe and effective use of medicines at all times.
5. Organise and manage the clinical pharmacy service for assigned wards, units, or clinical teams.
6. Offer clinical pharmacy advice, aiding in the management of patients to maximise medicinal benefits while minimising risks.
7. Participate in and support clinical audits to uphold the highest standards of pharmaceutical care.
