Are you an experienced clinical pharmacist who is passionate about advancing clinical pharmacy services within primary care? The Melksham and Bradford on Avon Primary Care Network (PCN) is looking to recruit a Clinical Pharmacist to join their established pharmacy team and enhance our clinical pharmacy service at a practice level, working over the two practices based in Melksham (Spa and Giffords). This is an exciting opportunity to be part of an experienced team of Pharmacists and Pharmacy Technicians. We are a network covering over 48,000 patients in Wiltshire working across 3 surgeries. An established team of pharmacists who are innovative and committed to delivering high quality care for our patients. We work closely with our community pharmacy colleagues and have successfully launched GP-CPCS, now Pharmacy First, made significant changes to our medication review process and ways of working.
Main duties of the job
* Complete the CPPE Development Pathway.
* Perform structured medication reviews (SMRs) to proactively manage patients with complex polypharmacy and care home residents.
* Be involved in the management of long-term conditions.
* Embrace service development to grow their capabilities and the clinical pharmacy team.
The ideal candidate will be a Pharmacist with significant clinical experience and being an independent prescriber is desirable. We are looking for someone with leadership qualities, who is committed to improving patient care and is passionate about clinical pharmacy. The candidate must be able to work effectively as part of the practice team and be comfortable in encouraging new ways of working. Although prior experience in General Practice would be an advantage, pharmacists from other sectors with transferable skills are welcome.

Job responsibilities
The post holder will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines management, provide face to face structured medication reviews, manage long-term conditions, management of medicines on transfer of care and systems for safer prescribing, manage repeat prescription authorisations and reauthorisation, acute prescription requests, while addressing both public health and social care needs of patients in the GP practices that make up the PCN.

Qualifications
* GPhC registered pharmacist.
* Flexibility to work outside of core office hours.
* Disclosure Barring Service (DBS).
* Evidence of continuing professional development.
* Hold a GPhC independent prescribing qualification.
* Minor ailments certification.
* Membership of the Royal Pharmaceutical Society.
Experience
* Minimum of two years working as a pharmacist demonstrated within a practice portfolio.
* An appreciation of the nature of GPs and general practice. An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing.
* Experience and awareness of common acute and chronic conditions that are likely to be seen in general practice.
* In-depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
* Understanding the mentorship process.
* An appreciation of the new NHS landscape including the relationships between individual practices, PCNs and the commissioners.
