Job Summary
Mayflower Medical Group is looking to recruit an ACP/ANP to join our clinical team.
The Advanced Clinical Practitioner (ACP) plays a pivotal role in enhancing the level of care provided to patients through advanced practice interventions. This role involves autonomously managing complete episodes of clinical care across all age groups and demographics, encompassing a wide range of clinical conditions, diagnosing, treating, and referring patients as appropriate. The ACP operates within a multi-disciplinary team and is instrumental in mentoring and supporting both nursing staff and trainee practitioners.
Main Duties of the Job
Responsibilities:
1. Patient Care: Deliver advanced and complex clinical care, providing assessment, diagnosis, treatment, and prescribing for patients with acute, chronic, and complex conditions.
2. Autonomous Practice: Work independently within the scope of practice to manage complete clinical episodes, using advanced diagnostic skills and clinical judgment.
3. Case Management: Manage patient cases from initial assessment to conclusion, including follow-ups and referrals, ensuring continuity of care.
4. Prescribing: Utilise prescriptive authority responsibly following current guidelines and protocols to enhance therapeutic outcomes for patients.
5. Home Visiting: Undertake home visiting as and when required, including triage of home visits.
6. Mentorship: Provide mentorship and supervision to nursing staff, trainee advanced practitioners, and other healthcare professionals to enhance team competencies.
7. Service Development: Lead the development and implementation of new clinical services and pathways, improving processes and patient care.
8. Professional Development: Engage in continuous professional development to keep abreast of medical advancements and to further develop clinical skills.
9. Record Keeping: Maintain comprehensive and accurate patient records using SystmOne.
10. Meetings: Attend and contribute to team meetings, providing updates and insights on patient care strategies and outcomes.

The Fuller and Forbes Partnership has recently acquired the Mayflower Medical Group, which has practices currently in Leeds, Devon, Burnley, and Gateshead, providing care to over 120,000 patients. The Mayflower group, based in Plymouth, provides care to 35,000 patients across 5 locations: Stirling Road, Mannamead, Trelawny, Mount Gould, and Ernesettle. The surgery comprises a diversified clinical team consisting of GPs, ANPs, ACPs, a nursing team, and a full PCN team with clinical supervision and mentorship by GPs to medical students.

Person Specification
Essential Criteria:
1. Registration with the Nursing and Midwifery Council (NMC) or Health and Care Professions Council (HCPC) or other regulatory body as appropriate.
2. Masters degree in advanced clinical practice or active and up-to-date accreditation through the transition route.
3. Independent and supplementary prescribing qualification.
4. Extensive experience in managing complete episodes of clinical care including assessment, diagnosis, treatments, and prescribing.
5. Excellent communication skills, with the ability to convey complex information to patients and team members effectively.
6. Experience in a leadership role, providing mentorship and guidance to less experienced staff.
7. Competence in using SystmOne for effective record-keeping and data management.
