We are seeking full-time applicants; however, part-time hours will also be considered. A valid driving license and access to a vehicle for work purposes are required.
This position is open only to applicants from the Hampshire & Isle of Wight NHS system.
Main duties, tasks & skills required
To provide a high standard of clinical care, encompassing a holistic approach under the direction of a trained nurse, in patients' homes.
Working within a defined community nursing caseload for a group of GP Practices as part of the Integrated Community Care Team.
The post holder will maintain safe practice by following relevant Trust policies, ensuring holistic nursing assessments and care plans are recorded in line with Trust and NMC standards.
The post holder will work with a high degree of autonomy and professional accountability. A primary aim of the role is to prevent unnecessary admission of patients to an acute hospital and support timely discharge into the community.
