Job summary
An exciting opportunity has arisen for a motivatedenthusiastic Registered nurse to work as a part of the rheumatology services.To provide information, advice and support to patients with a rheumatologicalcondition who are being monitored, until established on DMARD therapy.
The role requires running own clinics and also givingtelephone advice on an ad hoc and rostered basis, recognising when onwardreferral/advice is needed. The nurse has a pivotal role in supportingmultidisciplinary teams and coordinating the pathway for patients. This is adevelopmental role and there is an expectation that the post holder willcontinue their own professional development in line with personal anddepartmental requirements.
Main duties of the job
The post holder will be responsible for assessing,monitoring and reviewing patients with a rheumaological disease and who arereceiving Disease Modifying Anti Rheumatoid drug (DMARD) therapy including oralbiologic treatment.
To ensure that all patients receiving DMARD treatment aresafely monitored in a timely manner, and according to local and nationalguidelines, and transferred to shared care as soon as is safely possible.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

To assess patientsneeds implement & evaluating programmes of care following local andnational standards policies care pathways and protocols in particular NICE& BSR British Society of Rheumatology recommendationsTo workautonomously during clinic time assessing and monitoring patients includingblood tests who are taking disease modifying drug therapy and reporting anyabnormalities to a more senior member of the rheumatology team
Participatein patient education and promote self-management
Report anddocument any relevant patient needs concerns with other health careprofessionals who have ongoing responsibility communicating changes as theyoccur
Assistwith the telephone advice line being aware of own limitations in managing patientproblems and referring on as appropriate but using each intervention as anopportunity to develop own knowledge and skills
Demonstratea commitment to evidence based practice and clinical excellence and play anactive role in using this evidence to improve the patients experience
Takeownership of database so that information readily available regarding DMARDactivity within the service. Ensure this information is also available to thewider rheumatology team
Adhere toNMC Code of Practice standards and record keeping at all times
