Role Summary

Are you interested in working in a supportive community environment and seeking your next challenge? We are looking for an innovative, enthusiastic, and motivated Associate Community Matron to join the Urgent Community Response team, working flexibly across the Tiverton and Cullompton Cluster. We are an integrated health and social care team focusing on crisis management, preventing hospital admissions, and supporting timely discharges, helping people remain as independent as possible at home. This fast-paced team offers daily learning opportunities and new challenges. This position is ideal for an experienced Band 5 looking to progress or a Band 6 clinician seeking a new challenge. The successful candidate should be committed to providing compassionate, holistic patient care.
Working Pattern: 37.5 hours per week. The service operates 7 days a week, 8 am to 8 pm, on a rota basis.
