Job summary
The Discharge to Assess pathways supports individuals to return to thecommunity from hospital with the appropriate level of support based on need.This need can include reablement to help promote the individuals to return totheir previous level of independence and wellbeing utilising Enhanced RecoveryBeds. This multi-disciplinary approachis based within the Community Urgent Care Team Therapists, also workingalongside Adult social care partners in Focus and the Community Stroke team.

Base: Westgate Park, Charlton Street Grimsby, DN31 1SQ. Maybe required to work within North East Lincolnshire.
Up to hours per week Until March 2025
Salary Range 4 £24,645 - £27,202per annum, you will be appointed a fixed point within therange
The successful applicant will co-ordinate betweenthe multi-disciplinary agencies to organise the delivery of services to supportthe individual in a timely manner whilst in an Enhanced Recovery Bed. The role also involves liaising with individuals,their families and Professionals to help optimise a persons outcomes includingreturning their home.
The role will involve working closely withmulti-disciplinary teams and working autonomously when necessary, and providing theadministrative service to overview an individuals recovery, as well asmaintaining the qualitative and quantitative data collections to review andimprove the service.
