We are looking for full time and part time Practitioners to join Thistle’s Wellbeing teams located in Midlothian or East Lothian GP Practices.
1. Are you a health and social care practitioner with experience of supporting people living with Long Term physical and mental Health Conditions to self-manage?
2. Do you cope well supporting vulnerable people who experience significant psychosocial barriers and health inequalities?
3. Do you believe people have strengths, are resilient and are experts in their own lives?
4. Do you thrive working autonomously in the community setting?
5. Do you have good solution focused coaching and group facilitation skills?

The post holders will:
6. Work with a team of practitioners to deliver our Midlothian or East Lothian GP contracts
7. Be prepared to travel across East Lothian to engage with people referred to the Wellbeing Service. (see further information on location below)
8. Practice persistent engagement to help people who struggle to receive support
9. Manage a caseload of people referred to the service
10. Use a strengths based personal outcomes approach to help people identify and access appropriate support for them to self-manage including:
11. Providing specific coaching in self-management
12. Delivering group based Lifestyle Management courses
13. Linking people to community resources and supports
14. Support volunteers who have lived experience of long-term conditions into peer roles
Location: In East Lothian practitioners will be based in Musselburgh practices (Inveresk and Riverside) and/or Prestonpans and Tranent and significant travel between other GP practices across East Lothian is an essential aspect of this role. In Midlothian, travel to more than one GP practice is essential. We will consider interest in full or part-time hours.
