The Role:
We are looking for an experienced social worker to join the South Mental Health Team as an advanced social work practitioner (ASWP). You would be joining a well-established and passionate team supporting individuals and their carers experiencing severe and enduring mental health challenges. We support individuals from 18 years (including care leavers from 17.5 years) upwards throughout different stages of their life journey. We work closely with our colleagues in health to ensure a holistic and person-centred approach as well as our colleagues in commissioning and the voluntary sector.
Often our work with individuals is long term, which enables the opportunity to get to know the person you are working with well so that care planning can be creative and centred around the person's goals and ambitions. We also support people detained under the Mental Health Act in out-of-county hospitals or rehabs to support plan that person's discharge back into the community. When doing so, we will consider a range of options that could meet their needs and promote their recovery. We have dedicated hospital social workers attached to our local inpatient units, and as an ASWP, you'd work closely with them to support transfers once the person is discharged.
As a team, we are committed to supporting one another and have team members on various development pathways such as AMHP training, practice educator courses, apprenticeships, and areas of specialist interest which they are invited to feedback on in our monthly team meetings. We encourage regular reflection with fortnightly reflective practice sessions and have the social work academy that offers a range of training opportunities.
As an ASWP in the team, you would be responsible for the line management of social workers and social care workers. You would also partake in the 3-weekly Duty Manager Rota alongside the 2 other ASWPs and attend regular MDTs with the neighborhood mental health team that you will be aligned to. You may also hold a small complex caseload and undertake social supervision of individuals under Ministry of Justice restrictions.
