Contract type: 12 months (fixed term)
Hours: Full time - 37.5 hours per week (this role is shift patterns over a 24 hour period)
Salary: £35,392 - £42,618 per annum
Grade: NHS AfC: Band 6
Job Overview
The Home Treatment Team (HTT) provides a 7 day a week service, between the hours of 8am and 10pm, whose key responsibilities are around the assessing and delivery of care for people who are in crisis with their mental health. The team is multi-disciplinary and has a strong ethos of team working and team support, in order to deliver high quality care.
Principal Duties
* Triage and determine urgency of new referrals to the service
* Carry out comprehensive assessments of an individuals health and social care needs in a variety of settings and agree the most appropriate course of action
* To assess, evaluate and manage clinical risk
* To formulate a plan of care based on identified need
* Deliver evidence based crisis interventions as appropriate
* Review effectiveness of care plan at regular intervals
* To work collaboratively with service users, carers and partner agencies
* To coordinate the clinic work of a day by day basis
* To maintain computer held care records in an accurate and timely fashion
* Undertake all work in a culturally sensitive manner
Qualifications
Essential criteria:
* Registered Nurse Mental Health
* Experience of working as a Band 5 Nurse in a MDT setting
* Evidence of continued professional development
Knowledge & Skills
Essential criteria:
* Working knowledge and understanding of Care Programme Approach & Care Management
* Knowledge and understanding of appropriate therapeutic approaches
* Demonstrable risk assessment and risk management skills
* Ability to work autonomously as well as a co-operative and effective member of the multi-disciplinary team
Experience
Essential criteria:
* Experience of multi-disciplinary team working
* Experience of working in a community setting
* Experience of mentoring others
