Key duties To promote and maintain the positive well-being of the people who live in services provided by Choice Care by assisting in the provision of Positive Behaviour Support for individuals supported by the organisation. To uphold high standards of Positive Behaviour Support in line with policy and practice. To develop and maintain a special interest in recovery-based approaches and support for people with learning disabilities and / or mental health difficulties. As guided by the Clinical Lead and the Senior Practitioner to undertake assessment, formulation work alongside individuals in the context of their history and relationships. This will occur within a broad framework of Positive Behaviour Support and Recovery based thinking. To co-ordinate on going behaviour monitoring of challenging behaviours in relevant services. To assist in the development of interventions aimed to reduce the intensity, frequency and potential challenge of behaviours that are challenging to the individual and the service. To offer clearly specified individual and group work to people who receive Choice Care Group services under the supervision of the Clinical Lead and or the Senior Practitioner. To facilitate the smooth and efficient implementation of the Care Plan systems within the homes. Personal attributes A full UK driving licence and access to a car is essential as you will be visiting the services and homes on a weekly basis. The company will contribute to the mileage for work related travel.
