You will have post registration experience, preferable, in the specified care group, and will be someone who can make a significant contribution in supporting the delivery of high quality assessment, investigation and treatment, supporting the team in the day-to-day running of the service whilst being expected to take a lead on a core aspect of service delivery. The post holder will work with individuals who present with a range of memory related health needs, by providing assessment, information, treatment, review and support through individual and group work as part of a multi-disciplinary team Main duties of the job You will work as a Care coordinator for a group of patients, and be responsible for liaising with other professionals and organisations to ensure that patients' physical, social and psychological needs are met in an integrated and holistic way.
