MMWF is a growing PCN (Primary Care Network) which has four GP surgeries. All the surgeries are within the Northampton town. We have a diverse population and having someone who can speak multiple languages will be helpful but not essential.
We are looking for a qualified Dietitian to join our team and help us provide nutritional advice and guidance to our clients. The successful candidate will have a strong knowledge of nutrition and health, as well as an understanding of the principles of diet planning. The Dietitian will be responsible for creating individualised meal plans, providing nutrition education, and monitoring the progress of clients. We are looking for someone who is passionate about helping people achieve their health goals and is willing to go the extra mile to ensure our clients' success.
Main duties of the job
An exciting opportunity has arisen for an enthusiastic HCPC registered Dietitian with excellent communication skills and self-motivation.
The role will require the candidate to have a valid UK drives licence and access to a car. This post requires autonomous working to provide pharmaceutical care to our patients. You will have daily appointments to offer medication reviews and medication advice as well as minor alignments and other contractual obligations including QOF and IIF schemes

This post would be ideally suited to an experienced dietitian wishing to take the next step in their career progression seeking a new challenge. This role will build on your existing dietetic skills - clinical knowledge, communication skills, motivational interviewing, caseload management and collaborative working.

Primary Care Network working alongside The Mounts Medical Centre, Maple Partnership, Favell Plus Surgery and Mayfield Surgery.
The successful applicant will join our friendly and supportive PCN. The post will cover a wide range of clinical conditions including management of nutrition support, Diabetes (type 1 and type 2), weight management, dysphagia, food allergies and intolerance, Gastrointestinal and eating disorders / disordered eating.
Previous experience of working with a variety of nutritional needs.
Responsibilities:
1. Educate patients on nutrition and healthy eating habits.
2. Develop individualized meal plans for patients.
3. Monitor patient progress and adjust meal plans accordingly.
4. Conduct nutritional assessments and provide counseling services.
5. Keep up to date with the latest nutrition research and trends.
6. Develop recipes and menus that meet nutritional guidelines.
7. Work with other healthcare professionals to coordinate care.
