East London NHS Foundation Trust, Mental Health Care of Older People team, is looking for an enthusiastic and proactive individual to provide patient case management to improve discharge flow of City and Hackney Older adult patients.
The successful individual will be part of a community and ward based team consisting of nursing, therapy, social work, and admin team members. You will be working closely with the inpatient, health and social care partners to ensure safe and timely discharges and minimise barriers, improving patient experience across discharge pathways.
You will also contribute to the discharge planning of older adults admitted into General hospital or working age inpatient wards who need support to be discharged.
You will be part of a wider community team delivering a discharge to assess model of care in the community. You will be supported by the CMHT Operation lead, Senior Social Worker Practitioner, Senior Lead Nurse, Senior Clinical lead OT, and will work closely with system partners including acute staff, local authorities, voluntary agencies and the ICB.
Main duties of the job
You will work as a Discharge Social Worker, representing a social work perspective to older adults experiencing mental health difficulties.
As a Discharge Social Worker, you will be a champion of the social model of intervention within the discharge process from Acute Inpatient Wards and General hospital Wards and will ensure that social factors contributing towards the mental health difficulties are recognised and addressed as far as possible.
You will be expected to work in partnership with service users and carers, identifying and utilising strengths and working closely with other agencies and voluntary groups to ensure good outcomes which, as far as possible, maintain independence.
You will have a good working knowledge of The Care Act, Mental Health Act and Mental Capacity Act/DOLS and other social care legislation. Within the team it is vital that Social Workers including the Discharge social worker maintain a social perspective to their work and promote social care issues within the team.
As part of your role, you will offer expert advice and guidance to the inpatient colleagues on adult safeguarding matters, including writing reports and representing/supporting conferences and other relevant meetings.
You will complete post discharge 7 day follow up and 6 week review prior to handing case over to the community Care Coordinator.
