Ashfield Adult Supported Accommodation provides temporary accommodation for individuals that need some assistance before moving into long-term, independent accommodation.
We work with people 18 plus whom, Ashfield District Council have a homelessness Duty of Care for. Individuals will have their own self-contained flat with staff support available during the hours of 08:00-20:00 Monday – Friday. Outside of these hours will be covered by Concierge & duty manager service.
We have 17 core flats situated at Outram St and 5 flats at Penn St. The Aim is to move people on quickly into their own accommodation. You will support people with life skills, working with specialist teams to provide a supportive wrap around support enabling empowerment and autonomy.
We require a Support Planner for 23 hrs per week. We work a continental shift pattern consisting of 2 x 12 hr shifts 08:00-20:00. A Concierge service will cover 20:00 – 08:00 and weekends are currently covered by staff and relief workers.
We are looking for a candidate who can communicate with people in a friendly and welcoming manner. We are looking for a resilient, reliable, self-motivated candidate who can demonstrate an understanding of the issues facing those affected by homelessness. An awareness of the support needs of our client group is essential. Experience of working in a supported housing setting or similar setting is desirable.
The main function of the role will be:
• To complete individual support plans using Salesforce – setting SMAT targets and reviewing monthly
• To assess individual service user’s skills and needs and to work positively with risk including risk management.
• To ensure safety and security of the clients and the building
• To build the skills and confidence of service users to empower them in maximising their independence
• To contribute to the achievement of contractual service obligations
• To carry out domestic tasks around the service to maintain the health and hygiene of the physical environment
• To work in partnership with other agencies to maximise service delivery
