Role Overview: We are seeking a dedicated and professional Social Worker to join our team. The successful candidate will provide a strength-based, outcome-focused assessment service to adults across community and hospital settings. This role is integral to promoting independence, enabling recovery, and safeguarding adults. Key Responsibilities: Deliver a professional social work service in line with statutory social care responsibilities and organizational policies. Contribute to service delivery that emphasizes strengths-based and outcome-focused approaches. Collaborate with other professionals and agencies to address complex needs and achieve optimal outcomes for individuals and their carers. Conduct high-quality assessments and develop support plans in consultation with individuals and their carers. Prepare and present written reports in various formats as required. Work with health colleagues to ensure safe and timely discharges from hospital to appropriate services and settings. Promote the creative and flexible use of personal budgets to meet the needs of individuals and their carers. Communicate effectively with people, carers, other agencies, and professionals to support coordinated responses to needs. Manage and prioritize a caseload effectively. Maintain accurate records using electronic record systems (Liquid Logic) and other relevant ICT systems. Contribute to team working and service development, identifying improvements to enhance efficiency and effectiveness. Support and mentor newly qualified or less experienced staff and students. Engage in continuous professional development, including mandatory e-learning and formal training. Maintain emotional resilience and professional wellbeing while developing practice competence and confidence. Participate in a duty system, working flexibly and responding to urgent situations as necessary. Undertake additional duties as deemed appropriate by the team manager.
