Kent Community Health NHS Foundation Trust
We are looking for a skilled Nurse to join our Pathway 3 Service based around Maidstone and Tunbridge Wells.
Pathway 3 supports discharge to local care homes for individuals who require a further period of assessment to determine the long-term plan of care when an acute hospital admission is no longer required.
We work in collaboration with the acute trust Discharge Liaison Teams, Kent County Council Social Services Teams, All Age Continuing Health Care, local community services, and organisations including the voluntary sector.
Main duties of the job
In this role, you will be visiting patients in their homes and assessing their needs to support the development of a long-term care plan.
The community-based nature does mean that a driving licence with access to a car is essential.
Communication and compassion are fundamental parts of the role; liaising with partner organisations, patients, relatives, and carers is vital to ensure processes are understood and supported by all involved in the individuals' care and assessment.
You will be joining an "Outstanding" rated Trust, with a commitment to patient care, our workforce, your well-being, and your development.
Additional Working Pattern: 8-6 (4-day weeks, including approx 1 in 5 Saturdays)
About us
Rated 'outstanding' by the CQC, we have a passion for delivering high standards of patient care and excellent services to improve the health of our communities. To do this, we need outstanding people who share our values: compassionate, aspirational, responsive, and excellent.
We know a commitment to equity, diversity, and inclusion leads to better patient care and a happier workforce. We pride ourselves on being diverse and welcome applications from people with varied backgrounds, perspectives, and experiences.
We ask for information about your protected characteristics on our application forms. This isn't visible to anyone involved with shortlisting or interviewing you, except when you choose to be considered under the guaranteed interview scheme due to disability. Short listers will see two ticks alongside your application to ensure they apply the principles of the scheme to your submission.
The data we collect helps us ensure we are attracting diversity in our applicants, that all our colleagues have equitable access to career opportunities, and supports us on our journey to being a Great Place to Work.
As a flexible working-friendly organisation, we want you to be able to work in a way that is best for us, for our patients, and for you. Talk to us about a flexible working arrangement that won't involve sacrificing time for personal interests or family commitments. We'll support you to work flexibly in a way that suits us both.
Job responsibilities
