Job summary We are looking for a proactive and forward-thinking Practice Nurse Manager to support and enhance our team of nurses and HCAs. You will be joining a friendly and supportive team, and will be supported by the GP partners and practice management team to ensure that we are able to deliver outstanding care to our patients. This is a part-time role (24-28 hours per week). Main duties of the job The role will involve management of our nursing team, leading the team, responding to changes in nursing practice. Clinical responsibilities will include: Assess, plan, develop, implement and evaluate programmes which promote health and well being, and prevent adverse effects on health and well being. Assess, plan, develop, implement and evaluate individual treatment plans for patient with a known long term condition Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long term condition as appropriate. Lead on wound care management, participate in immunisation programmes for both adults and children. Lead on Nursing staff appraisals, day to day management of the nursing team. Lead on infection control. About us We are a small and friendly GP surgery, based on the outskirts of Chester city centre. We provide care for 8,000 patients and are led by 4 supportive GP partners. Our practice team also includes 2 salaried GPs, 3 ANPs, 1 additional Practice Nurse, 3 HCAs, physiotherapist, dietitian, child and adult mental health practitioners, care co-ordinator, and a large administration team.
