Are you a Prescribing Pharmacist with Primary Care experience? If so, my client would be delighted to have a conversation with youBased in Durham, this position would be 3 days at one site, and 2 days at another site in the same Primary Care organisation, about 18 miles away from each other.Similarly, if you would prefer to be PT, they can split the role and accommodate 2 Pharmacists accordingly.Salary to be confirmed however, IRO £43,000 - £50,900p/a or Pro Rata depending on training and experience. Key Duties Perform medication reviews and optimisation by conducting comprehensive medication reviews for patients to ensure safe, effective, and appropriate use of medications. Prescribe and re-prescribe medications in accordance with clinical guidelines, patient needs, and organisational policies. Manage long-term conditions by supporting patients with chronic diseases (e.g. diabetes, hypertension etc) by reviewing and optimising treatment plans. Liaising with the MDT, communicate with hospital pharmacists or specialists to discuss complex cases or medications that require specialised knowledge.
