We are looking for a dedicated and highly motivated team player to join our welcoming, experienced, and proactive Hospital Team in Social Services.
The team has an emphasis on an integrated approach, working creatively to achieve positive outcomes, ensuring that care is delivered in a dignified and respectful way. You will work with Social Workers, Occupational Therapists, other Social Care Assessors and partners in the NHS to ensure the most appropriate care and support is in place for a person to leave hospital, and that the most up to date equipment, technology and techniques are being used. You will give information, advice and guidance whilst helping to promote wellbeing and independence and preventing, reducing, or delaying the need for services. When necessary, you will ensure that the people using our services move seamlessly through to other teams whilst recording information using the IT systems in place. Therefore, the ideal candidate will require skills such as having a professional manner, being a skilled communicator, being organised and having prior good computer literacy. You will receive training on specific IT systems used.
Our Social Care Assessors play a critical role in supporting people to develop creative and innovative solutions to meet the outcomes they want to achieve.

You will have:
* NVQ Level 3 in Health and Social care or equivalent (Desirable).
* Be required to undertake an enhanced DBS plus Adults Barred List (Essential).
* Experience of working with vulnerable people and their carers.
* Ability to communicate effectively with customers, carers and families.
* Awareness of importance of multi-disciplinary working and an integrated approach.
* Problem solver with good planning and organisational skills.
* Ability to relate to other disciplines in a sensitive and professional manner.
* Some knowledge of assessment and support planning within legislation.
* Ability to cope positively with change and an ability to promote positive changes to make a difference.
* You are confident, energetic and enthusiastic as well as sensitive and resilient.
* Self-motivating individual with the passion to make a difference in adult social care.
* Resilient personal stress management.
This is a hybrid role where you will be based at our offices across the borough as well as the opportunity to work at other locations, complete visits and work from home.
