Role Overview
– Support vulnerable adults, promote independence, and well-being through collaboration with individuals, families, and professionals.
Key Responsibilities
– Conduct assessments and develop personalized support plans.
– Respond promptly to safeguarding concerns, ensuring legal compliance.
– Provide emotional support, advocacy, and facilitate group sessions.
– Collaborate with healthcare professionals and agencies, advocating for clients’ rights.
– Maintain accurate records and ensure compliance with data protection guidelines.
Qualifications and Experience
– Degree in Social Work.
– Registration with Social Work England.
– Experience in adult social care.
– Knowledge of relevant legislation.
