An exciting new development of a Care Coordination Hub requires a Clinical Team Manager.
The CTM will line manage the administrative team who are based at the hub and manage the day-to-day activity delivered by the clinical staff, ensuring organised, safe and professional triage and coordination of patient care.
The CTM will be responsible for appraisal, training updates, absence management and recruitment of the administrative team.
As a CTM, you will engage in the Governance of the Hub including data collection, efficiency and effectiveness of the Hub, safe staffing, development, education and incident reporting and management.
You will need to be an experienced band 6 or 7 who has insight into community teams, District Nursing, Recovery at Home and Palliative Care who is prepared to lead change and development.
There is an opportunity for this post to be considered for a 12 month secondment, only if discussed and agreed by the applicant's line manager at the time of interview.
Main duties of the job
The CTM will be responsible for the safe and organised running of the Hub; this will include daily huddles, safety huddles, safe staffing, break allocation, incident reporting and management, professional communication to patients and professionals, efficient and effective working, staff support and training.
The implementation of new ways of working will be mindful that staff are rotating into the role, so support and guidance will be given.
Collection of data and ensuring staff are updated about the Hub activity.
To be able to work across services ensuring communications are shared with appropriate managers and staff.
The role will require the CTM to work mainly Monday to Friday between the hours of 07:30 and 20:30, 52 weeks of the year. Some variation of shifts may be required for audit, support and development.
If you are clinical, an exception will be that you maintain your clinical skills with agreed clinical time allocated.

Job responsibilities
* To be accountable for the overall management and development of the service, deployment and supervision of staff, ensuring the coordination of the hub is provided, optimising the cost-effective use of all resources.
* Line management of the administrative team, including call handlers and administrative staff with overall accountability and responsibility for the delivery of all aspects of the Community Coordination Hub, through the provision of effective clinical and managerial leadership.
* To lead, motivate and support the team approach to delivering triage and coordination models of care.
* To lead a continuously improving service, by ensuring systems are in place to support clinical governance, quality monitoring and ongoing research and audit of practice.
* Ensure systems and processes are in place to maintain patient and staff safety.
* Facilitate the development of administrative staff and support rotating clinical staff in new ways of working and development within the hub.
* To work autonomously, assessing any staff or patient incidents linked to triage whilst working at the hub. Share any learning and developments linked to any incidents.
* To act as a resource for all members of the Multi-Disciplinary Team, providing expert advice concerning aspects of patient/client management.
* To maintain own clinical competencies by agreeing clinical days with your line manager/Matron.
* To be responsible for ensuring that agreed professional standards are reflected in practice and to have overall responsibility for monitoring the standard of care.
* To improve the patient/client journey by increasing access to triage, assessment and appropriate care and treatment allocation.
* Work collaboratively with Multi-Disciplinary Teams to ensure practice is efficient, effective, evidence-based and safe.

Physical Skills
* Able to move and handle safely without causing harm to themselves or others.
Experience
* Significant experience at Band 6 level or above in a community setting.

* Portfolio of evidence of continuous professional development.
* Experience of leading clinical development.

Qualifications
* Registered Nurse/AHP.
* Degree Level Training.
* Post registration qualification specific to role.
* Masters in health/leadership related subject.
* Teaching qualification.
