Job summary A vacancy has become available within the Dementia Assessment Service South team which covers the Adur, Arun East, Worthing and Chanctonbury area. This is a rare opportunity to become part of a person-centred and dynamic team as a Community Support Worker. Possession of a valid UK car driving licence is desirable but not essential. Main duties of the job The successful candidate will be supported to deliver the following: Support medical staff in clinics by conducting information gathering to aid the assessment process. To carry out post-diagnostic reviews either by telephone or in person and liaise with a qualified member of the multidisciplinary team following the review. Under guidance of the multidisciplinary team, in a time limited manner and where care planned to do so, carry out clinical visits to offer 1:1 intervention to aid adjustment to diagnosis, management of memory difficulties and engagement in meaningful activities to improve quality of life. This will involve visiting service users in their own homes and in other community settings. To provide people under our care information to help them meet their own needs and signpost to other community services that will assist them in remaining independent. To liaise with other professionals and agencies based on an individual's needs. There is also the potential for opportunities to help with the administration, organisation and co-delivery of dementia specific groups.
