Job summary Our Crisis Resolution and Home Treatment teams are multi-disciplinary and provide a community based mental health service. CRHTT aim to provide a safe and effective home-based assessment and treatment service as an alternative to in-patient admission. We can facilitate early discharge from the ward which provides an opportunity for patients to receive care and support in their own homes which is the least restrictive environment. The workload is team-based and our team members do not carry a specific caseload themselves, instead they share one as a collective unit. The teams are resourced to provide care and brief crisis intervention 7 days a week for people who are experiencing a mental health crisis. The Service support adults from the age of 18 which is recovery focused and work in collaboration with clients whilst supporting carers dynamic according to individual need. The response will include assessments at MHA's and home visits. Staff will undertake rapid assessments and then either refer or signpost to other services. Out of hours activity will be coordinated through the Mental Health support Line and responded accordingly. Merton Crisis Resolution and Home Treatment Team offer an alternate to acute psychiatric admission and the team acts as 'Gatekeepers' to the acute ward and the team works closely with the Trusts in patient's wards to help action this effectively. Main duties of the job We are currently looking for full time Nurses Practitioners Band 6, who are motivated and committed to a be part of high-quality services working within a multi-disciplinary team including recovery support and peer support workers. We are looking for practitioners who will be committed to delivering high standard of patient care which is evidence based. The successful candidates will be based at either Springfield Hospital in Tooting or The Wilson Hospital in Mitcham and will be expected to work a combination of early and late shifts which will include weekends. In addition to the benefits of working part of a multi-disciplinary team, the right candidates will be working mostly independently. You must be able to have good communication skills, be organised and possess good time management. A car driver/owner is essential.
