OCES supports elders and adults with disabilities by providing vital information and coordination of services that promote healthy and safe lives for our consumers.
We are seeking a caring Registered Nurse to facilitate successful discharge activities and other transition support activities as a member of our Transition Support Team. In addition this role will work with our Home Care Team to assess and/or authorize services for our consumers.
What you will do:
* Assist with discharge planning and transition support activities; collaborate with OCES staff and appropriate community resources.
* Conduct visits to all Skilled Nursing facilities in OCES' area to review Consumer Lists with discharge planners and meet with consumers.
* Determine consumers' MassHealth clinical eligibility for nursing facility services; determine duration of short-term approvals; determine eligibility for other transition support programs and activities.
* Track consumers after discharge per program/contract requirements
* Determining consumers' eligibility for programs and services according to procedures and guidelines
* Establishing, reviewing and revising care plans in accordance with funding source regulations, OCES procedures and standards
* Other duties as assigned.
What you will bring:
* Bachelor's degree in Nursing, Social Work or related field preferred.
* Current Massachusetts RN License
* Two years of experience in the social services field including a minimum of one year of experience with transition support activities.
* Considerable knowledge of developing assessment care plans.
* Excellent organizational skills and attention to detail.
* Proficiency with Microsoft Office Suite or related software.
