To assist the portfolio Community Lead to ensure the safe delivery of care through the development of community pathways, support, and interventions within the assigned portfolio. To be responsible for the operational management, and delivery of community services within the assigned portfolio, ensuring the provision of high quality, safe and responsive services within the available resources. To manage and support the development of patient-centered clinical practice and best standards of clinical care liaising with colleagues and external agencies as appropriate. To manage and support community staff in relation to performance, recruitment, appraisals, and continual professional development. To support the development of the community workforce, through early identification of the necessary skills and knowledge required to support the strategic direction of BDCT portfolios.
