We are partnering with a forward-thinking North London borough to recruit a Social Worker for their Reablement Service. This short-term, up-to-six-week service is designed to help residents regain independence and rebuild confidence. Your Role: As a key member of a multidisciplinary team, including home support workers, therapists, pharmacists, and case managers, youll provide holistic care tailored to each clients unique needs. Your goal will be to empower individuals, ensuring they achieve maximum independence during their reablement journey. Key Responsibilities: The Reablement Social Worker will lead and coordinate personalised reablement plans for clients with complex needs. Conduct assessments, including Mental Capacity Assessments, in line with the Care Act 2014 and safeguarding policies. Manage a diverse caseload, balancing risk management with respect for clients' lifestyle choices. Collaborate with colleagues across health and social care to ensure seamless service delivery. Provide expert advice and guidance to team members, stakeholders, and external partners. Maintain accurate, up-to-date records and contribute to service development through multi-disciplinary meetings. What Were Looking For: Availability to work flexibly across a 7-day schedule to meet service needs. Solid knowledge of safeguarding, the Mental Capacity Act 2005, and relevant legislation. Experience conducting holistic assessments and creating plans that promote independence. Strong communication, organisational, and risk management skills. Additional Expectations: Engage in training, supervision, and team meetings to support professional development. Uphold values: quality, equality, and empowerment. Investigate safeguarding concerns and provide detailed reports for legal and professional purposes. Apply diverse social work methods, including counselling and protection, to meet client needs.
