We specialise in the provision of UK wide community nursing and hospice care, both inpatient and outpatient, whilst providing information and support on all aspects of terminal illness, dying, death, and bereavement. Our leading research pushes the boundaries of what we know about good end-of-life, and our campaigns fight for a world where everyone gets to have the best possible quality of life while living with an illness they’re likely to die from.
The care and support we provide is highly valued by the people we care for and their loved ones, but at present we are only reaching around 10% of dying people at the end of life. Right now, one in four people in the UK with a terminal illness do not get the care or support they deserve at the end of their lives.
We are recruiting for a Senior Nurse to join our Community-based team in Dorset. As a Senior Nurse, you’ll act as a manager of a team of Registered Nurses, Nurse Associates, and Healthcare Assistants, as well as supporting the Clinical Nurse Manager.
This is a Dorset Community based role across Christchurch, Bournemouth, and Poole, which will see you work from home and out in the community.
Salary aligned to AFC band 6 - £35,392 - £37,350
In this role, you will:
* Deliver hands-on care on a regular basis as part of the clinical team.
* With support from the Clinical Nurse Manager, undertake My Plan and Review / CDPF for nurses within the immediate team, and/or support the registered nurses to undertake appraisal of more junior staff. This will result in the creation of individual development plans to be overseen by the Senior Nurse.
* Facilitate the development of nursing practice through specific projects in line with the place-based objectives.
* Act as a managerial and clinical resource for services within their remit.
* Co-ordinate patient activity and encourage a wide team approach to the provision of patient care.
* Regularly oversee care to ensure staff are providing the highest standard of care and that any training or development needs are assessed and supported.
* Actively participate in own clinical supervision and promote the clinical supervision of junior staff.
* Deliver and participate in formal and informal teaching to reflect the needs of the service.
* Regularly participate in observations of clinical practice to monitor standards of practice and create bespoke developmental plans for staff within their team.
* Support the CNM with investigations into incidents and complaints.
* Ensure that the voice of the user is heard and support with facilitating high quality patient and carer feedback.
* Regularly co-ordinate the service including allocation of staff, responding to queries, and liaising with the coordination hub regarding referrals and allocations.
Key skills and experience:
* Registered Healthcare Professional on NMC register.
* Experience caring for patients with palliative care needs.
* Collectively reflect on and assess the impact of person-centred ways of working within the multidisciplinary team.
* Understand benefits of working with a patient-centred philosophy of multidisciplinary team working.
* Incorporate knowledge of life limiting conditions and best practice evidence into care planning and decision making.
* Experience of formal and informal teaching.
