The role of the colorectal cancer support worker will be to work with the colorectal clinical nurse specialists to support patients undergoing a colorectal cancer diagnostic pathway, patients with a cancer diagnosis, or patients living with and beyond cancer. The colorectal cancer support worker will develop key working relationships internally with the Colorectal Team, Multi-disciplinary team, and tertiary cancer centres, as well as with outside stakeholders such as Macmillan, patients and families, and patient support groups.
Main duties of the job
The colorectal cancer support worker will work closely with clinicians, specialist nurses, administration staff, and all members of the Multi-disciplinary Team to ensure that patients are actively managed and supported throughout the cancer journey. Responsibilities include:
1. Support with the delivery of personalised care.
2. Provide support and escalate recurrence of cancer through a single point of contact and cancer wellbeing programmes.
3. Assist with risk stratification and delivery of open access patient follow-up for colorectal cancer patients, which includes requesting surveillance bloods and facilitating timely imaging and endoscopies.

Job responsibilities
To work with and cross-cover single point of contact and re-entry contact number. Responsibilities include:
1. Coordinate the follow-up investigations required post-cancer diagnosis or treatment and provide a point of contact for patients post-cancer treatment.
2. Promote patient empowerment and self-management strategies.
3. Deliver self-management advice, support, and know when to escalate to the clinical team.
4. Support and coordinate the EHNA tracking, ensuring the monitoring of patient needs.
5. Responsible for coordinating rapid re-entry for specialist review against a clinically approved triage model.
6. Provide fast and accurate clinical correspondence administration service with proficient use of appropriate software and systems to meet the required Trust's standard of work.
7. Coordinate and organise appointments and information.
8. Arrange face-to-face and telephone communication and engagement with cancer patients and families.
9. Ability to deal with level 1 distress and recognise and escalate concerns regarding psychological wellbeing and management.
10. Involvement in the planning/organisation and delivery of health and wellbeing events and supported management events.
11. Development of survivorship and wellbeing services.
12. Support the department and organisation by carrying out any other duties that reasonably fit within the broad scope of a job of this grade and type of work.
