Job Purpose
To provide high standards of care to patients with health and social care needs. Care will be delivered within the patient’s own home or other community settings with an enablement focus where appropriate. The post holder will be required to deliver care under the supervision of senior support workers or therapists, following a prescribed care plan and reporting back to other members of the team, the patient, and family/carers as appropriate, highlighting any concerns or changes in the patient’s condition or abilities. The post will involve lone working. Senior support workers and therapists will provide necessary support, and there is access to on-call provision out of hours.
1. To communicate professionally and effectively, both verbally and non-verbally, with patients and their carers. This may include communicating with patients and carers who are deaf, visually impaired, have poor memory, cognitive problems, or for whom English is not their first language.
2. To use skills of persuasion, motivation, and explanation to encourage patients and carers to participate in enablement tasks where appropriate, which may be painful, challenging, and/or time-consuming, and therefore require negotiation.
3. To convey appropriate information to patients and carers in a sensitive and timely manner using language they understand.
4. To work within the team maintaining close liaison with all Quickstart/Maximising Independence staff to ensure the smooth running of the team and the effective delivery of the service to patients.
5. To communicate professionally and effectively, both verbally and non-verbally, with colleagues and other health and social care professionals, voluntary agencies, and carers to expedite continuing care of patients.

To carry out pre-defined care plans with patients in their homes, as directed by a senior support worker or qualified therapist for a range of patients, promoting an enablement ethos where appropriate, ensuring that patient safety and care is prioritised at all times.
To report back to the allocated senior support worker, clinician, or therapist, and document effectively care tasks undertaken, progress made, outcomes of intervention, and any concerns regarding patients' health, wellbeing, or ability, and to discuss when modification of care plans may be necessary as part of a structured clinical supervision system.
To utilise basic health observation techniques to monitor patients' health status as appropriate and directed by senior staff, reporting back findings.
To ensure all aspects of work are carried out to a high clinical standard and be accountable for this through the senior staff.
To maintain confidentiality on all matters relating to patients/families adhering to Trust policies.
Communicate effectively with patients, families, and other members of the multi-disciplinary team to ensure the prescribed plan of care is delivered.
