Main duties of the job This is a community-based role that requires frequent travel to various locations throughout the county including areas that may not be accessible by public transportation. Successful candidates will need to demonstrate how they will manage these travel requirements. To be responsible for the assessment of care needs and clinical management of a defined caseload within the Cornwall Partnership NHS Foundation Trust (CFT) Mental Health Service. To carry continuing responsibility for the development, implementation and evaluation of programs of care, utilising the care co-ordination system. To develop the interface between primary and secondary care services so the patients/carers are offered focused and co-ordinated packages of care that are appropriate to their needs You will need to have knowledge of the CPA process, legislation around mental health and The Mental Health Act. You will need excellent communication skills and place value on team and inter-agency working.
