Due to the retirement of our long-standing Practice Nurse, Dale Medical Practice is looking for a Part-time Band 5 equivalent nurse to support our existing clinical team. Salary commensurate with experience.
The role is for 20-25 hours per week. Practice experience is desirable.
Main duties of the job
The postholder will work within guidelines and protocols to undertake treatment room nursing duties and assist with nurse-run long-term condition clinics. Candidates should have change-management skills and the ability to support patients in changing their lifestyle. Good IT and communication skills are essential.

Dale Medical Practice is a forward-thinking GMS Training practice with 6500 patients. We have 3 GP Partners, 1 Salaried GP, 1 ANP, and Practice Nurses. Together with PCN ARRS staff, we have a FT GPA and SNA and access to Pharmacist, Dietitian, Physio, Mental Health Practitioners, Social Prescriber, Health and Wellbeing Coach, and support from Care Coordinators. Job responsibilities
KEY RESPONSIBILITIES
Working within guidelines and protocols undertake and record:
1. Urinalysis and preparation of specimens for laboratory analysis
2. Temperature, pulse and respiratory rate
3. Weight, height, waist circumference, and Body Mass Index
4. Measurement of legs for compression hosiery
5. ECGs, ambulatory blood pressure monitoring
6. Near patient testing e.g. blood glucose, lipids, and INR
7. Doppler ultrasound measurement working with registered nurse
8. Measurement of airway function (peak expiratory flow rate)
9. Obtain venous blood samples from individuals for investigation.
10. Administer medications by injection with the appropriate legal authorisation.
11. Recognise health promotion opportunities and give brief, focused lifestyle advice using motivational interviewing techniques which may include:
12. Weight management
13. Sexual health
14. Undertake initial assessment of patients presenting with wounds, working within expertise to manage independently or refer to other services as necessary.
15. Remove wound closure materials including sutures, clips, and staples.
16. Take samples for the cervical screening programme according to recognised standards.
17. Administer vaccines to adults and children according to the UK immunisation schedule.
18. Provide a pre-travel health service including administration of vaccines and advice after undertaking appropriate risk assessment.
19. Administer prescribed therapies such as contraceptive injections, vitamin B12, and depo neuroleptic medications recognising side effects and contraindications.
20. Develop and follow referral pathways for patients with specialist needs.
21. Assist nurse-run long-term condition clinics:
22. Carry out clinical measurements and near patient testing for patients with diabetes, asthma, and COPD in line with NICE guidance and Quality and Outcome Framework indicators which may include:
23. Assessment of feet with appropriate risk stratification
24. Pulse oximetry
25. Peak Expiratory Flow rate
26. Teaching inhaler technique
27. Assessment of breathlessness.
28. Recognise signs and symptoms that may indicate undiagnosed long-term conditions e.g. poor wound healing, winter cough in smokers, and also factors that may indicate mental health problems and use relevant referral pathways.
29. Participate in quality improvement and innovations e.g. audit, significant event analysis.
30. Contribute to the development of protocols and new services that aim to reduce unwarranted variation as outlined in the framework for nursing, Leading Change, Adding Value (May 2016).
31. Attend in-house meetings and contribute to the development of the nursing service and where appropriate wider practice systems.
32. Participate in ongoing supervision and mentorship of unregistered health care staff, nursing students, and junior nurses including mentorship for preceptor programmes and contributing to the revalidation needs of nurse colleagues.
33. Attend in-house and external training events and contribute to the continuing education of unregistered health care staff by identifying learning opportunities.
34. Complete statutory training/updating and mandatory training as defined by the practice.
35. Complete skills-specific training and regular updating.
36. Participate in an annual individual performance review maintaining a record of own personal and professional development including the requirements for revalidation with the NMC.
37. Take responsibility for own development, learning, and performance. Identify gaps in knowledge and skills and how further learning can enhance delivery of nursing service.
38. Identify and address risks in work activities working with the practice nurse manager and practice manager.
39. Follow appropriate infection control procedures.
40. Maintain work areas in a tidy and safe manner.
41. Communicate effectively with individuals in other agencies to meet peoples needs.
