Job summary
We are looking for a practice nurse to join our nursing team.
We have a variety of skill mix and are keen to ensure we appoint the right person into our team irrespective of whether this will be equivalent to a band 5 or 6.
We would ideally like someone with an interest and/or experience in respiratory medicine but this is not essential.
Main duties of the job
The main duties of the role will be providing a patient facing nursing service to the practice population. The role is expected to include but not be limited to:
Chronic disease management (including diabetes, respiratory, hypertension)
Womens health
Vaccination and immunisation programme (including seasonal vaccinations)
About us
We are a large cohesive team serving a population of 25,000 over 3 sites. We have a team of 20 doctors, 7 nurses, 1 nursing assistant, 2 healthcare assistants and a phlebotomist. The nursing team has an overall lead and meets regularly as a nursing team as well as meeting with the whole clinical team. All opinions of team members are heard and valued with a view to help shape the service we provide.
We strive to provide high quality patient care. To that end we often have regular educational opportunities at the practice, group and personal level. This includes opportunities to share good practice and reflect on personal and patient needs.
We are passionate about supporting and developing the while team and whilst we work hard we are careful to support one another and have done work on wellbeing for the whole practice team. We believe that effective and open communication is paramount and promote this where possible

1. Act independently within the practice team
2. Manage own clinical practice and that of othermembers of the nursing team to ensure safe effective care
3. Accept delegated responsibility and inherentaccountability for the quality or own practice
4. Assess, plan and evaluate clinical nursing careto meet the needs of the individual and/or identified group
5. Assess, diagnose and treat specific conditionsin accordance with agreed professional protocols and guidelines
6. Take the clinical lead in defined areas ofnursing practice within the primary health care team.
7. Monitor own and teams work areas and practicesand ensure they are free from hazard and conform to safety legislation.
8. Receive direct referral from health careprofessionals, other agencies and patients
9. Plan, manager and deliver programmes to improvehealth and disease in individuals or groups.
