This position may be scheduled at any of the following sites: Charlton Campus (Hamilton Downtown), King Campus (Stoney Creek East Hamilton), West 5th Campus (Hamilton Mountain), Brantford General Hospital Satellite Hemodialysis Unit (Brantford), Satellite Health Facility (Hamilton Downtown), Six Nations Satellite Hemodialysis Unit (Ohsweken)

We are looking for a pharmacist to join our Pharmacy Services team!

The ideal candidate enjoys engaging with patients, has the clinical and disease-management skills to identify and resolve complex drug therapy problems, and is comfortable working to their full scope of practice.
This position is an opportunity for a pharmacist to take on the role and responsibilities of a clinical pharmacist, including to identify, prevent and resolve drug therapy problems, develop and document therapeutic plans, and optimize medication selection, dosing, and monitoring by working in a fully electronic computerized physician order entry health information system. In addition, you will have the opportunity to work in a collaborative team environment, participate in hospital and departmental projects, and teach learners, pharmacists, and students.

QUALIFICATIONS:
1. Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD) from an accredited university required
2. Registration with the Ontario College of Pharmacists, Part A, in good standing
3. Injection and Immunization Certification from an OCP-recognized program preferred
4. Previous hospital experience is an asset
5. Hospital Pharmacy Residency preferred
6. Certification or advanced training in disease state management is an asset
7. Demonstrates strong customer service orientation and skill working collaboratively with others
8. Experience working on or leading quality improvement initiatives is an asset
9. Possesses excellent oral and written communication skills
10. Excellent attendance and punctuality are essential
RESPONSIBILITIES:
11. Review medication orders for safety and appropriateness
12. Provides customer centric care, recognizing both patients and other internal stakeholders (eg: other healthcare providers) as customers
13. Provides pharmaceutical care to patients while working under full expanded scope of practice
14. Ensures optimal patient care by identifying, preventing and resolving drug therapy problems
15. Oversee tasks performed by Pharmacy Technicians to ensure safe and accurate medication dispensing
16. Precepts and mentors students and residents
17. Attend and present at staff education seminars
18. Performs duties in a manner that demonstrates the employee’s commitment to developing a culture of patient and staff safety through accountability, reliability, trust and team work.
19. Uses professional communication and excellent interpersonal skills to work effectively with colleagues within and outside the department
20. Effectively manages daily workload using demonstrated ability to prioritize patients and tasks
21. Identify and follow up on adverse reactions and medication incidents
22. Contributes to the corporate efforts and initiatives, as appropriate, to enhance SJHH’s mission, vision and values, by participating in departmental and hospital projects, committees and task groups as assigned
23. Acts as a liaison between Pharmacy Services and other healthcare provides (eg: nurses, allied health staff and physicians) to provide education and to promote pharmacy initiatives
24. Maintains confidentiality and privacy standards at all times
25. Adheres to all applicable NAPRA/OCP regulations and hospital/Pharmacy Services policies and procedures
26. Performs all duties as assigned by Manager
NOTE: This position will be mainly at Charlton Campus, but may be scheduled to work at other campuses as well.
