Leaving Care Specialist (Personal Advisor)

1 x full time role – 37 hrs, 1 x part time role – 18.5 hrs
Our client is seeking a Personal Advisor to join a Children’s Services Team based in Bradford. You will be required to support young people who are in care or have recently left care to maximize their life opportunities and reach their full potential.
Responsibilities:
* Direct and group work with young people
* Facilitate and monitor the assessment of need
* Facilitate Pathway Plans based on assessments to address needs
* Liaise with others involved to ensure that assessed needs of the young people are met
* Provide practical, personal and emotional support to children and young people
* Promote their health, physical, emotional development and wellbeing
* Provide support and advice on health, benefits, education, training, employment, accommodation/housing and personal well-being
* Identify barriers to learning and help overcome these
* Develop personal action plans with the young people
* Review and evaluate progress of young people
* Advocate on behalf of young people who do not have the skills to do this
* Work alongside colleagues both within Social Services and from other agencies, and engage with significant people in the young person's life
* Transport young people where necessary
* Keep and maintain records and write reports
* Attend meetings, reviews and court where necessary
Requirements:
* Relevant NVQ Level 4 qualification or equivalent
* Driving License, access to a vehicle, business insurance
* Previous experience working with young people leaving care
* Experience of housing, employment and training issues for young people
* Experience of working in a supportive relationship with young people
* Understanding of voluntary and statutory agencies and an awareness of opportunities within the Local Community for young people
* Enhanced Child and Adult DBS registered to the update service
