You will be required to work within the wider multi-disciplinary team providing evaluating and communicating effectively with other teams within the Service. Main duties of the job To work as part of a team to provide comprehensive clinical support to the Service. To provide care and support to the patients and families within the Unit. Under the direction of registered health care professionals, carry out holistic care, according to the patients' treatment plan. In this way, fully engage with a multidisciplinary approach, where the contribution of registrants from nursing and the different health professions are recognised and deployed appropriately in the best interests of patient care Support patient care through provision of key skills including emotional and spiritual care, positioning, communication, tissue viability, oral and personal hygiene, and nutritional awareness. Assist the patients to meet personal hygiene needs according to the treatment plan, maintaining their comfort and dignity at all times. Report any changes in patients' condition or care needs to the supervising trained nurse or relevant therapist and document accordingly. Assist patients move in accordance with safer handling plan. Maintain accurate and complete patient records in accordance with trust guidelines ensuring countersignature by registered nurse. Be aware of patients' emotional, physical, and social needs and always perform duties in such a way as to preserve patients' dignity and freedom of choice.
