An exciting opportunity has arisen within the Triage and Assessment Team for a registered Mental Health Nurse to come and join at a time when we are evolving for a fixed term of 12 months.
The role will include completing triage and assessments with clients with a range of mental health issues, ensuring clients receive the most appropriate assessment of their needs and ongoing referral to treatment/support services. Working collaboratively with locality team staff to ensure the best outcomes for clients.
Your team base will be at St Andrews Place, Hull, and cover Hull and East Riding. You will be part of a supportive team of staff and will have the opportunity to gain skills and experience in the triage and assessment of clients. You will also work closely with colleagues within Community Mental Health services to ensure clients are given a collaborative approach to their care and assessment.
Key Responsibilities:
1. Complete triage and assessments with clients.
2. Ensure appropriate referrals to treatment/support services.
3. Collaborate with locality team staff for optimal client outcomes.
4. Work closely with Community Mental Health services.
