We are looking for a Senior Referral Clinician, who will work within the Single Point of Access team at HCRG Care Group, ensuring a high standard of service delivery.
As a Senior Referral Clinician, you will be responsible for the digital screening of referrals to ensure effective prioritisation, safety, and accuracy. This involves making clinical decisions regarding the eligibility for access to services, applying a no wrong door approach to ensure inclusivity and seamless care pathways.
The role focuses on managing complex referrals, including those involving safeguarding concerns and intricate health needs, ensuring a robust and safe response to the needs of children and families.
The successful candidate will bring clinical expertise and substantial experience in children's health and development.
As a Senior Referral Clinician, you will clinically supervise and inspire the team to pursue excellence in care delivery, while providing clinical guidance and day-to-day leadership to uphold the quality and safety of services.

* Discussing appropriate care pathways with referrers and families to understand their current needs and expectations of the service.
* Supporting clinical and day-to-day leadership of the team including induction and development of new colleagues.

Job responsibilities
* A qualified Nurse or Allied Health Professional e.g. RCN, SCPHN, SLT, OT, Physio or equivalent, and clinical registration with a recognised professional body (e.g. NMC, HCPC).
* Postgraduate/post-qualification experience working with Children and Young People for a minimum of 3 years.
* Excellent communication skills, able to communicate with families, young people, and other professionals verbally and in writing.
* Exceptional clinical risk assessment and management skills, with a sound understanding of current evidence and best practice in child health and care.
* Minimum of 1 year experience providing clinical supervision to others.
* Experience of working as a senior member of an MDT.
* A positive, motivated, and passionate approach to providing high-quality holistic referral management.
