Associate Community Matron- West Waverley
The Associate Community Matron will work with the Community Matron Team to deliver proactive patient-centred care across Guildford and Waverley, collaborating with other health, social, and voluntary services.
This role focuses on developing expert clinical skills for managing long-term conditions, ensuring patients receive appropriate care as close to home as possible. Responsibilities include making connections with acute and community hospitals to reduce unnecessary admissions and facilitate timely discharges. The post holder will work across the local Primary Care Network area under the direction of the Community Matron to ensure equity of care.
Main Duties of the Job
The post holder will work as part of the Community Matron Service under the supervision of a Community Matron and in conjunction with the wider multidisciplinary team (MDT) to:
1. Assist in the proactive identification of adults at high risk of unnecessary hospital admissions.
2. Work as part of the wider MDT, including GPs, adult social care, mental health, community nursing, and geriatricians.
3. Utilise a proactive and anticipatory approach to assessment in the home setting.
4. Develop proactive care plans with patients, carers, and families to reduce risks and promote self-management.
5. Deliver new and improved pathways for individuals with long-term conditions and complex needs.
6. Utilise the electronic clinical record-keeping system, EMIS, for documentation, with a provided work iPhone and iPad for mobile working.

Job Responsibilities
Key Responsibilities:
1. Work collaboratively to ensure good relationships across the Primary Care Network.
2. Communicate effectively with GPs, practice nurses, community nurses, social services, and other stakeholders.
3. Utilise effective communication skills to overcome barriers to understanding.
4. Act as an advocate for individuals and carers, prioritising their needs and preferences.
5. Maintain accurate documentation and clinical records in line with professional and organisational requirements.
Person Specification
Qualifications
* Registered nurse with current NMC registration.
* Evidence of study at BSc / level 6.
* Willingness to undertake study at Level 7 Postgraduate in Advanced Practice.
* Evidence of recent and relevant continuing professional development.
* Management / Coaching / CBT / Quality Improvement / Leadership training.
Knowledge and Experience
* Recent experience in Primary or Community care.
* Broad range of clinical nursing skills, including venepuncture, catheterisation, injections, and wound care.
* Clinically up to date in Chronic Disease Management.
* Understanding of political factors influencing community nursing services.

Salary: £39,205 to £47,084 a year Pro Rata Per Annum inc. HCAS
