Job summary
We are looking for a TriageAdministrator to provide high quality, compassionate care to our patientpopulation. We are a large practicedelivering care to a total of around 26,000 patients across four sites.
You will need a caringand compassionate nature, excellent organisational skills and to enjoy working with people. The world of General Practice is busy and ever changing so an ability tolearn quickly and be adaptable to rapid change is essential. The role willrequire an ability to organise and prioritise your own workload, and so you should be comfortableworking independently and as a committed member of a multi-disciplinaryteam.
Anexciting opportunity has arisen to join this team as a Triage Administrator.This role is to screen andprocess all incoming Footfall and email contacts in a timely manner to helpprovide high quality,compassionate care to our patient population. Additionally to provide seniorguidance for all patient contacts and to be a pivotal role liaising between allteams in the practice delivering a comprehensive range of administrativesupport to Triage Clinicians.
Main duties of the job
To triage incomingFootfall or email contacts from patients
To direct clinicalqueries to the Triage Clinician or other AHPs, and administrative queries tothe correct member of the admin team
To respond directlyto queries either via email or via Footfall when inside your competencies
To work alongsidethe Pharmacists, Mental Health Worker, Social Prescriber, First ContactPractitioner as well as GPs and nurses to ensure patients are directed to themost appropriate member of the practice team
To work alongsidethe practice partners and practice manager to design and deliver serviceimprovements to the online triage model
To assist inservice development through evaluation, audit and system research
To haveresponsibility for administrative co-ordination of incoming patient queries
Maintain accurate,contemporaneous record keeping of all interventions with patients, fullyutilising computer software systems in place and/or other mediums.
You will demonstrateexcellent communication skills and will act as first point of contact for thepractice
Promote healtheducation to our patient population, such as through participation inself-care week
Support the practicesto deliver on, QOF, KPIs and locally commissioned enhanced services
To update andmaintain accurate patient medication records
