Salary: $25.96 - $28.85 commensurate with experience.
Employment: This is a part-time (16-20 hours per week) position with the possibility to grow to a full-time (30-40 hour per week) position. Days and hours of work schedule will be 2-3 days per week Monday – Friday during business hours (variable each day). Evening work may be required as job duties demand.
Required Education and Experience:
* Master’s Degree in Social Work or Psychology from an accredited program.
* Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW). LCSW preferred.
* Clinical experience working with individuals with developmental and acquired disabilities during education or work experiences in the field is preferred.
* Must have excellent oral and written communication skills.
* Must have demonstrated excellent listening skills.
* Must have a valid NYS Driver’s License.
* Computer proficiency.
* Must have demonstrated commitment to performing job duties and tasks at a high level of performance.

Summary/Objective: Responsible for the evaluation and treatment of clients with developmental and acquired disabilities referred to CP Rochester for counseling services. Functions as a member of a multi-disciplinary team providing counseling services in accordance with individual needs and treatment plans. The focus is on maximizing the potential of the individuals in all facets of their life and providing needed support to families and caregivers.
Essential Functions:
* Imparts agency philosophy of “Equal Opportunity, Independence and Realization of Individual Potential” to clients, families, program staff, and community resources.
* Maintains confidentiality of information relating to individuals and their families in accordance with agency policy.
* Provides counseling services, based on medical referral, including initial evaluations, treatment planning, treatment sessions, and re-assessment.
* Develops psycho-social assessments/histories and/or social/emotional development assessments, which note areas of strength, areas of weakness, and coping mechanisms of the individual.
* As a member of a multi-disciplinary team, identifies emotional and social problems of individuals and families and mobilizes. Can advocate for individuals in regard to community resources and services to ensure the individual receives the maximum benefit of the services.
* Documents all diagnostic assessment reports and treatment notes accurately and in a timely manner, and provides such information to treatment team as needed.
* Maintains documentation that meets the standards of the agency, regulatory agencies, and the standards of care.
* Consults with and provides training for agency employees regarding the social and emotional needs of clients.
* Adheres to department, agency, regulatory agency, and professional policies, procedures, guidelines, and standards of care.
* Attends meetings as assigned.
* Participates in conferences, workshops, and in-service training as needed or recommended to enhance job skills and personal growth and development.
* If LMSW, attends monthly supervision with licensed Psychologist or LCSW.
* Achieves program and department goals by performing additional responsibilities as required/assigned.

Travel: Is primarily local during the business day but the employee will be expected to work at any of our locations. Some out-of-the-area and overnight travel may be expected.
