Job summary Interview date for this post will be Tuesday 26th November 2024. This is a new role within the PCN Pharmacy hub and the Enhanced Health in Care Homes team. The Clinical Pharmacist will support the Lead Pharmacist & Pharmacy Technicians in the PCN Pharmacy Hub team to work with the PCN and GP practices to deliver a programme of medicines optimisation including managing hospital discharge medication changes, secondary care implemented medicines and practice medication queries. It is anticipated that initially the successful applicant will predominantly support the Pharmacy Hub to cover long-term absence. The post holder will be responsible for encouraging the development of a better understanding of the principles of medicines optimisation for the population of Darlington. There will also be a requirement to support PHD's long established Care Homes team with the work they do with medicines optimisation and support to Care Homes with medicines management. There will be a requirement to support MDTs for Care Home residents to support proper management of residents in relation to best use of medications. Main duties of the job Be an independent prescriber or commit to completing training to become a prescriber. Contribute to the delivery of effective daily processes for the transfer of care between hospitals, GPs and care homes including effective signposting, recommendation of medication changes and communication accordingly Monitoring creatinine clearance for DOACs and checking correct doses which will include changing appropriate patients to edoxaban. Support colleagues and patients with medication queries and monitoring. Undertake clinical medication reviews to proactively manage people with complex polypharmacy. Focus especially on the elderly, people in care homes, those with multiple co-morbidities (frailty, COPD and asthma) and people with learning disabilities or autism. Provide specialist expertise in the use of medicines, while helping to address both the public health and social care needs of patients. Using audit and action planning, help to tackle inequalities. Provide leadership on person-centered medicines optimisation (including ensuring prescribers in the PCN practices conserve antibiotics in line with local antimicrobial stewardship guidance) and quality improvement. Contribute to the General Practice quality and outcomes framework (QOF) and enhanced services. Use structured medication reviews to support patients to take their medications to get the best from them, reduce waste and promote self-care.
