Please see the full job description for more information KEY DUTIES:Professional responsibilities / Development To work in accordance with current local and national protocols and guidelines. Maintain and develop knowledge by participating in educational opportunities including online training, peer to peer forums and attending educational events relevant and necessary to professional development. To have the ability to work on own initiative, acknowledging limitations and seeking advice or referring to other colleagues or health care professionals where needed. To keep up to date with the changes within primary care nursing and be able to evaluate and adapt care using evidence-based practice. Participate in an annual individual performance review and maintain a record of professional development in accordance with the NMC revalidation process. Clinical Duties To include, but not limited to:- o Administering prescribed injections o Prescribing within defined remit o Childhood and Adult Immunisations in accordance with Patient Group / Specific Directions (PGDs / PSDs), or as prescribed. o Performing / interpretation of ECGs. o Monitoring and management of Blood Pressure, Blood Glucose and Cholesterol. o Review of ring pessaries. o Contraception counselling including assessment and review of oral, patch, ring, injectable and intrauterine devices. o Emergency contraception advice. o Collection of specimens - phlebotomy, urinalysis, swabs. o Cervical cytology, in accordance with national screening programme. o Assessment and syringing of ears. o Performing a Travel Heath risk assessment, administering recommended immunisation, arranging antimalarial prophylaxis and providing general health care advice and information. o Wound management. o Minor injuries management. o Assisting with minor procedures - operations / contraceptive devices. o Carry out diagnostic Spirometry and interpretation of results. o Perform Doppler studies, interpretation and management of the results. o Health checks / assessment - New Patient, NHS, Severe Mental Illness, Learning Disabilities, Memory tests (GP COG). o Health Education - smoking, alcohol, diet, exercise, weight management. o Health Promotion - promoting healthy lifestyles, well person checks, CVD risk assessment (Q risk) and encourage patient participation in national screening including Breast, Cervical, Bowel. o Long Term Condition reviews (to appropriate level) - Diabetes, Cardio / Cerebral Vascular Disease, Peripheral Vascular Disease, Heart Failure, Hypertension, Asthma / COPD. To recommend medication changes / modifications in accordance with agreed protocols / guidelines and current regulation. To provide telephone advice to patients / parents / guardians / relatives and occasional home visits where appropriate and within levels of competence. Any other duties as reasonably requested by the doctors / nurse manager providing these duties are within the scope of the role and that the individual has undertaken the appropriate training and is competent to undertake the duty. Professional Duties To keep accurate and comprehensive computerised medical records using appropriate read codes / templates. Ensure all documents relating to patient care are completed accurately i.e immunisation records / certificates. To access and file microbiology / cytology results using appropriate clinical information sources i.e Open Exeter / ICE. To work cooperatively in the provision of care to meet Quality Outcome Frameworks (QOF) and Enhanced Services (DES / LES). Refer patients to other services including Mental Health / Wellbeing, Drug / Alcohol, Weight / Exercise management, Smoking cessation, Podiatry, Physiotherapy, Occupational Therapy, District Nursing, Continence Service, Health Visiting and Leg ulcer / Chronic wound, COPD and Heart Failure management clinic. Understand the importance of effective communication within the team and strive to communicate effectively with other members of the practice, patients and their parents / guardians and carers. Ensure infection control procedures are followed and protocols are adhered to at all times. Any risk should be actioned and / or reported as indicated to the appropriate person. To be familiar with and follow protocol for the safe and effective running of the practice concerning all aspects of health, safety and security including data protection and fire safety and to undertake any recommended training. In participation with the nursing team ensure the maintenance of equipment, stock levels and of the safe running of the treatment room. Act accordingly to achieve the cold chain and maintain stock levels of vaccinations. In participation with the nursing team order vaccinations required and record daily fridge temperatures. Attend annual resuscitation training, be familiar with emergency equipment and drugs in the practice, and in participation with the nursing team maintain / check equipment and ensure drugs are up to date and equipment is fully functioning. Support, coordinate and participate in health promotion campaigns i.e Flu. Participation in annual influenza campaigns or other Public Health campaigns may include paid overtime at evenings and or weekends. Participate in practice meetings, audit and research projects. Assist with the collection of statistical information / data collection required to provide quality care. Participate in the education of students and the induction of staff of all disciplines (where appropriate). Effectively manage own time and work load. Maintain appearance and behaviour in keeping with a member of the nursing profession and in accordance with the code of conduct. Act in a way that supports the equity, diversity and rights of patients and colleagues. Maintain confidentiality and adhere to data protection policy whilst contacting other health care professionals / agencies in relation to patient care and practice matters. To be familiar with and follow the practice complaints procedure and seek advice where necessary. To be familiar with and comply with the practices clinical governance standards, policies and procedure. To attend recommended safeguarding training / meetings and keep up to date with local procedures and policies.
