Main duties of the job
1. Provide nursing expertise and palliative care advice in symptom control and psychological support for patients with advanced, progressing life-limiting illness with complex needs, and their families under regular supervision.
2. Act as an expert resource for patients, families/carers and health and social care professionals caring for them.
3. Support Advance Care Planning and other sensitive conversations utilising good communication skills.
4. Anticipate patient need, supporting prevention of avoidable crises within the context of understanding advanced disease, treatments and disease trajectories.
5. Promote collaborative working and personalised care and support 'what matters most' to patients and their families.
6. Manage a clinical caseload of patients under supervision and proactively support and co-ordinate the individualised care and support needs for patients and their families acting as patient advocate, through collaboration and participation with the rest of the multi-disciplinary team within primary and secondary care.
