You will work alongside our dynamic and welcoming team, along with MDT colleagues to provide ongoing support and care for service users. Main duties of the job The post holder is responsible for the overseeing of assessments of care needs and the development, implementation and evaluation of programmes of care for clients with Learning disabilities, Autism, Asperger's. The post holder works under the overall supervision of the Team Manager/Matron and is required to undertake the duties of the post without direct supervision. Active participation will be required in the training and education of learners allocated to the LD service, and will also encourage integration between the ward team and the community services.
